Collaborative Minds treats adults only. We are not a walk-in clinic or an emergency psychiatric clinic, but we do accept referrals.

Patient Screening Process

Becoming a patient at Collaborative Minds is easy, but due to the specialized nature of our care, the process does require just a little more than a phone call. On arrival as a prospective patient, you will fill out a questionnaire about your medical history, including your physical and mental health. Once this questionnaire is completed, our staff will review your case, and if you are accepted, we will begin the scheduling process for your first appointment. If you are not a good fit for our clinic, we are happy to suggest other clinics where you can receive care.
 
If this is your first visit, please bring the following information:
 
  • Identification (driver’s license, passport, or other photo ID)
  • Insurance card(s)
  • List of medications
  • Any previous medical history (if applicable)

Accepted Insurance

For insurance, we accept most major carriers, including Humana, Cigna, Aetna, Blue Cross Blue Shield, United Healthcare, Healthy Blue, and many other smaller plans as well. **WE DO NOT ACCEPT SECONDARY INSURANCE, MEDICARE, PEOPLES HEALTH, LOUISIANA HEALTHCARE CONNECTIONS, AETNA BETTER HEALTH OR AMERIHEALTH.**

Payment Methods

A $100 deposit is required for all appointments, which is applied directly to the cost of your co-payment. All co-pays are due at the time of your appointment, and we accept cash and all major credit cards (NO CHECKS). Cancellations must be made 24 hours before the appointment, or will incur a $100 no-show charge, which must be paid before any future appointments.
